Declaration
I declare that I have no interests, present or in the past in any entity in the tobacco or vaping product industries and I have not knowingly represented or received any contribution or compensation, directly or indirectly, whether financial or otherwise, from:
(a) any manufacturer, distributor, importer, or retailer of the industries;
(b) any parent, affiliate, branch, or subsidiary of any manufacturer, distributor, importer, retailer of the industries; or
(c) front group or any individual or entity, such as, but not limited to an interest group, think tank, advocacy organization, lawyer, law firm, scientist, lobbyist, advertising agency, business, or foundation, that represents, receives funding from, or works to promote the interests of the industries; this includes the Foundation for a Smoke-free World and Global Action to End Smoking.
Examples of interests that need to be disclosed are: consultancy/advisory, ownership of stocks and other forms of ownership interests, honoraria, gifts, or favours, travel cost coverage, contractual relationship, research funding, former or present employment, or being a beneficiary of such funding, direct or indirect funding, or any other interests not listed here that may be perceived to or give the appearance of potentially affecting your objectivity or independence.
I understand that strict compliance with this undertaking is a condition of my involvement with the CCTN25. Any registration found to be not in compliance will not be refunded.
I confirm that this declaration is to the best of my knowledge, true, accurate, and complete. Should there be any change to this information, I will promptly notify the Conference Secretariat by email on conference@cpha.ca and complete a new declaration of interest(s) form that describes the changes.
