NO CONFLICT OF INTEREST
I declare that I have no interests, present or in the past in any entity in the tobacco or vaping product industries and I have not knowingly represented or received any contribution or compensation, directly or indirectly, whether financial or otherwise, from (a) any manufacturer, distributor, importer, or retailer of the industries, (b) any parent, affiliate, branch, or subsidiary of any manufacturer, distributor, importer, retailer of the industries, or (c) front group or any individual or entity, such as, but not limited to an interest group, think tank, advocacy organization, lawyer, law firm, scientist, lobbyist, advertising agency, business, or foundation, that represents, receives funding from, or works to promote the interests of the industries; this includes the Foundation for a Smoke-free World and Global Action to End Smoking.
Examples of interests that need to be disclosed are: consultancy/advisory, ownership of stocks and other forms of ownership interests, honoraria, gifts, or favours, travel cost coverage, contractual relationship, research funding, former or present employment, or being a beneficiary of such funding, direct or indirect funding, or any other interests not listed here that may be perceived to or give the appearance of potentially affecting your objectivity or independence.
I understand that strict compliance with this undertaking is a condition of my involvement with the CCTN25. Any registration not in compliance will not be refunded.
I confirm that this declaration is to the best of my knowledge, true, accurate, and complete. Should there be any change to this information, I will promptly notify the Conference Secretariat by email on conference@cpha.ca and complete a new declaration of interest(s) form that describes the changes.

Je n’ai pas sciemment représenté ni reçu de contribution ou de rémunération, directement ou indirectement, qu’elle soit financière ou autre, de la part de (a) tout fabricant, distributeur, importateur ou détaillant de ces industries; (b) toute société mère, affiliée, succursale ou filiale de ces fabricants, distributeurs, importateurs ou détaillants; ou (c) tout groupe de façade ou toute personne ou entité, notamment un groupe d’intérêt, un groupe de réflexion, une organisation de défense, un·e avocat·e, un cabinet juridique, un·e scientifique, un·e lobbyiste, une agence de publicité, une entreprise ou une fondation, qui représente, reçoit du financement de, ou œuvre à promouvoir les intérêts de ces industries; cela inclut la Foundation for a Smoke-Free World et Global Action to End Smoking.
Voici des exemples d’intérêts qui doivent être déclarés : services de consultation ou de conseil; détention d’actions ou autres formes de participation; honoraires, cadeaux ou faveurs; prise en charge de frais de déplacement; relations contractuelles; financement de recherche; emploi antérieur ou actuel; statut de bénéficiaire d’un tel financement, qu’il soit direct ou indirect; tout autre intérêt non mentionné ici mais pouvant être perçu ou donner l’impression d’influencer votre objectivité ou votre indépendance.
Je comprends que le respect rigoureux de cet engagement est une condition de ma participation à la CCTN25. Toute inscription non conforme ne sera pas remboursée.
